Home Health is suitable for persons with varying needs. Patients must be 18 years or older – with some exceptions. Typical Home Health patients might include: An individual who has recently been discharged from the hospital and is not yet fully recovered; A person with a chronic condition that needs frequent monitoring, such as diabetes, kidney disease, stroke, heart / lung disease or multiple sclerosis; An individual with a terminal illness such as cancer or AIDS, who needs a program of pain management, emotional support and personal care; Someone with limited mobility, such as a stroke or fracture victim, or an elderly person who needs help with the activities of daily living; A person receiving intravenous antibiotic therapy at home rather than in the hospital.
