The Acute Rehabilitation Unit at Citrus Valley Medical Center – Queen of the Valley Campus provides acute inpatient rehabilitation services to patients recovering from amputations, stroke, brain injury, spinal cord injury, traumatic injuries and other neurological and orthopedic disorders.
Our goal is to provide optimum recovery of functional, cognitive, behavioral and psychosocial skills to facilitate the patient’s transition to home and an appropriate follow-up program.
Rehabilitation
Rehabilitation is the process of returning an individual to the most functionally independent and satisfying lifestyle that is possible following injury or disability. For many individuals it is a lifelong process that requires growing and changing within the individual and the family.

The Team
Members of the interdisciplinary team include: a medical director, consulting physicians, 24-hour nursing staff, physical and occupational therapists, speech-language pathologists, a social worker, a neuropsychologist, a case manager, a dietitian and an audiologist.
The staff of the unit functions as an integrated team, with the patient and family as the central members. The entire staff is involved in the continuity of care by planning and implementing individualized goals. Staff conferences are held frequently to ensure appropriateness of goals as the individual progresses.
The medical director, a physiatrist who specializes in physical medicine and rehabilitation, coordinates the activities of the entire treatment team and works with the varied medical consultants to ensure an integrated consistent direction for the patient’s rehabilitation program.
Rehabilitation nursing incorporates the patient’s medical problems, physical disability, cognitive limitations, and family structure into the care of the patient. This is accomplished through comprehensive assessment, planning and care delivery. Rehabilitation nursing focuses on skin care, bowel and bladder management, medication instruction, specialized health regiments and safety. Emphasis is placed on reinforcing skills the patient is developing in other therapy sessions.
Physical Therapy focuses on improving a patient’s functional mobility skills. This is done through ongoing evaluation and a vast array of modalities, exercise regimens and functional training approaches to improve performance in transfers, bed mobility, wheelchair use and ambulation.
Occupational Therapy retrains patients to perform activities of daily living such as bathing, dressing, grooming, hygiene, toileting and self-feeding by using specialized techniques or assistive devices. Home management skills and creative leisure or recreational activities may also be a focus treatment. Occupational therapy specializes in upper-extremity assessment and treatment to prevent complications and maximize function. Individualized treatment programs may also include perceptual-motor skills, training with assistive devices and home evaluations.
Speech-Language Pathology provides diagnosis, therapy and education for patients who demonstrate disabilities in speech, cognition, language, voice and swallowing. The speech-language pathologist conducts an evaluation to determine functional communication status including speech intelligibility, expressive language, language comprehension, voice, cognitive function and swallowing. An individualized treatment program is developed targeting deficits.
Neuropsychology provides evaluation of the patient’s intellectual and cognitive deficits and, in conjunction with the treatment team, plans and implements strategies to facilitate cognitive retraining.
Case Management coordinates and manages the care of patients to facilitate the achievement of quality outcomes. Case management works in collaboration with the interdisciplinary team.
Dietary Services monitors the patient’s nutritional status and recommends necessary dietary modification. Education to both the patient and family is incorporated.
Audiology Services provides an accurate evaluation of possible auditory deficits, which may contribute to further communicative deficits. The audiologist provides consulting with patients and families regarding the nature of the limitation and which form of amplification is appropriate. Avenues of habilitation/rehabilitation are presented.
Entry into the Program
Admission is based on a case-by-case evaluation of the individual’s need and potential to beneft from acute inpatient rehabilitation. If admission is not appropriate, every effort will be made to recommend alternate treatment placement.
Referrals to the Acute Rehabilitation Unit may be directed to the case manager at (626) 814-2457. All pre-admission screenings are completed within 24 hours of notification. Pre-authorization insurance is verified on all patients prior to admission to the Acute Rehabilitation Unit.
