Program-developed educational booklets to reinforce your time with the educators and to take home and review whenever you like—recently received a Special Achievement Award for one of our booklets. Physician receives written records / phone calls, as needed regarding your educational experience and continued progress. Beyond the Basics Significant cash discount. 30 - 60 minute follow-up every month for 12 months to provide either in depth information or continued support to achieve personal goals. Weight loss / management support In depth food label reading, menu planning, shopping tours and portion control demo Exercise, including demonstration of stretching, aerobic exercise, weight training at home and balance check Developing the change process, setting achievable goals, motivation and problem-solving Depression, anxiety, grieving, coping skills and relaxation techniques Diabetes and family issues Meter check, blood sugar goal progress and review of lab results Medication check and developing medication card Complications check, hypoglycemia and hypoglycemia unawareness Foot Check, including shoes and socks Reports / recommendations sent to physician on your progress monthly for a year for sustained lifestyle changes.
