| We serve nearly 1,500 new patients each year! A physician referral is required to enroll. We’ll contact your physician for a referral and authorization (if needed) to our program.
Basic Program
- 6 hours of basic diabetes education (3 sessions, 2+ hours each), additional advanced training and more as needed!
- Follow-up evaluations
- Two Registered Nurses, two Registered Dietitians, and two Marriage Family Therapists on site.
- Consultations / Classes are in small groups or individualized one-to-one, as needed for:
- Pre-diabetes, metabolic syndrome and hypoglycemia
- Type 1 diabetes, Type 2 diabetes
- Pregnancy and diabetes, including gestational diabetes, “pumpers”, and those planning a pregnancy
- Content of classes
- Nurse covers:
- What is diabetes?
- Methods to control diabetes
- Insulin, oral agent, and other injectable medication management
- Insulin injection instruction—learn the virtually painless method
- Insulin pump basic training, advanced training, and continued support
- Blood glucose monitoring—it doesn’t have to hurt
- Immediate and long-term complications—how to prevent
- Foot care
- Preventative maintenance and interaction with your physician
- Dietitian covers:
- Individualized meal plan
- Diabetic food exchanges and portions (including dining out and special occasions)
- Reading food labels
- Adjustments for losing weight or other health problems as needed
- Exercise guidelines
- Counselor covers:
- Adjustment to diabetes and motivation for diabetes self-care· Assertion training
- Stress management
- Lifestyle change and emotional / behavioral issues
- Community resources
- Private counseling sessions available for individual or family concerns
- Free support groups in English and Spanish for all types of diabetes and age groups.
- Free unlimited phone consultations to answer any questions you may have.
- Free umbrellas stroller to women enrolled in our Sweet Success Program.
- Program-developed educational booklets to reinforce your time with the educators and to take home and review whenever you like—recently received a Special Achievement Award for one of our booklets.
- Physician receives written records / phone calls, as needed regarding your educational experience and continued progress.
Beyond the Basics
- Significant cash discount.
- 30 - 60 minute follow-up every month for 12 months to provide either in depth information or continued support to achieve personal goals.
- Weight loss / management support
- In depth food label reading, menu planning, shopping tours and portion control demo
- Exercise, including demonstration of stretching, aerobic exercise, weight training at home and balance check
- Developing the change process, setting achievable goals, motivation and problem-solving
- Depression, anxiety, grieving, coping skills and relaxation techniques
- Diabetes and family issues
- Meter check, blood sugar goal progress and review of lab results
- Medication check and developing medication card
- Complications check, hypoglycemia and hypoglycemia unawareness
- Foot Check, including shoes and socks
- Reports / recommendations sent to physician on your progress monthly for a year for sustained lifestyle changes.
