We’re here for you! We have been helping people live with their diabetes for 15 years. The Citrus Valley Health Partners Center for Diabetes Education offers comprehensive diabetes education—we cover the basics and go “Beyond the Basics” to help you understand how to self-manage your diabetes. We are an American Diabetes Association recognized education program and an affiliate of the California Diabetes and Pregnancy Program (Sweet Success). Group classes and individualized one-to-one education cover nutrition, exercise, stress management, medications, monitoring your diabetes, preventative methods to maximize your health potential and much more! For moms-to-be and their babies, our Sweet Success program provides education prior to conception, throughout the pregnancy and postpartum follow-up. Our multidisciplinary team of nurses, dietitians, and marriage family therapists work with patients, their families and the community to help persons successfully manage this chronic disease.
Anyone can enter our program with a physician’s order. Patients from our Citrus Valley Medical Center and Foothill Presbyterian Hospital inpatient units may be referred to our Diabetes Education Program. Other people in the community living with diabetes also can take advantage of our Diabetes Education Program. The classes are offered Monday through Friday, by appointment in English and Spanish at two campuses: Foothill Education Center in Glendora and Citrus Valley Medical Center, Queen of the Valley Campus in West Covina.
