Dave Concepcion, MPT, CCCE, CSSGB
Corporate Director, Physical & Occupational Therapy, CVHP

Dave graduated from California State University, Long Beach with a Master’s in Physical Therapy with an emphasis in outpatient orthopedics. He is a Credentialed Clinical Instructor with APTA and has 13 years of experience in orthopedics, CARF-certified Acute Rehabilitation Unit, TCU and inpatient acute care. He serves as the center coordinator of clinical education focused in providing quality clinical experiences for graduate students in the physical therapy field. He has led rehabilitation initiatives in the comprehensive joint replacement program and developed protocols to maximize efficiency and outcomes of the joint replacement population in a Blue Distinction facility for orthopedic care. He has also been the keynote speaker in several population health conferences focusing in prevention of chronic disease states, including diabetes and joint arthritis. In 2016, Dave achieved Green Belt certification in Robust Process Improvement/Lean Six Sigma methodology to improve efficiency and quality in the delivery of health care services.


Glenda J. Fuentes, PT
Clinical Director, ARC Physical Therapy – Chino

Glenda “Gigi” Fuentes graduated from California State University, Long Beach with a BS in Physical Therapy. After starting the West Covina location in 1992, she has assumed the role of director in 2008 of the ARC Physical Therapy Chino office. She specializes in orthopedic injuries, pre- and post-op, sports injuries, manual therapy, shoulder and knee dysfunctions, foot and ankle injuries/orthotic casting and hand rehabilitation. Glenda’s competency is demonstrated in her day-to-day business of managing patients with a wide variety of basic and complex orthopedic conditions and achieving the most effective outcome. Over the years, she has advanced her professional career with continued educational courses geared towards orthopedic and sports-related conditions to benefit the specific patient population in our clinics. She shares her knowledge as a clinical instructor to our physical therapy students during their clinical education graduate programs.


Henry S. Moussa PT, MS
Clinical Director, ARC Physical Therapy – Glendora

Henry holds a Master’s in Physical Therapy from Azusa Pacific University. He specializes in sports injuries, manual therapy, total joint rehabilitation, shoulder and knee dysfunctions. Henry shares his knowledge and clinical expertise in orthopedic physical therapy by participating as a clinical instructor for physical therapy students. His specialized knowledge and advanced clinical proficiency to manage patients with complex orthopedic conditions and achieve the most effective clinical outcomes are some of his core competencies.


Jennifer Chavez, MPT
Supervisor, Physical Therapy, CVHP

Jennifer graduated from Azusa Pacific University with a Master’s in Physical Therapy. She has 15 years of experience in outpatient orthopedics, neuro (TBI, Stroke and spinal cord injury), pediatrics, general inpatient acute care, including acute rehab, TCU and cardiac care. She is a CCS paneled and Credentialed Clinical Instructor with APTA. Jennifer passes her passion down to the younger generations, working closely with nursing to provide education to nurses for joint replacement care and early mobility initiatives in a Blue Distinction facility for orthopedic care.


Ryan Jones, PT, DPT, CEAS
ARC Physical Therapy – West Covina

Ryan holds a Doctorate in Physical Therapy from Azusa Pacific University. He studied biomechanics of the throwing athlete with a focus on injury prevention with special interests in prevention of knee and back injuries, as well as functional movement analysis and body mechanics training. He brings nine years of experience in outpatient orthopedics, as well as acute TCU and Rehab settings.


Stevanus Ibrahim, PT, DPT
Citrus Valley Health Partners

Stevanus graduated from Western University of Health Sciences in 2008. He has eight years of experience in both outpatient and acute settings with a heavy focus in outpatient orthopedics treating surgical and nonsurgical conditions in children, geriatric and professional athletes. He specializes in prevention of knee and back injuries, as well as functional movement analysis and body mechanics training. His skills include kinesio taping, McKenzie Method both taught within the Institute of Physical Art for functional manual. In his time off, he enjoys golfing, fishing, lobstering and spending quality time with his twin daughters and his wife.


Melissa Concepcion, PT, DPT

Melissa graduated from Azusa Pacific University with a Doctorate in Physical Therapy. She has over 11 years of clinical experience in the outpatient adult and pediatric settings, as well as in the inpatient and acute rehabilitation areas. She is certified in the LSVT BIG program to treat Parkinson’s disease and related movement disorders. She is NDT trained and her special interests are working with clients with neurological conditions such as CVA, SCI and brain injury. Melissa is a Credentialed Clinical Instructor with APTA and is CCS paneled. Melissa enjoys community outreach, including promoting the physical therapy profession in local schools and health and wellness.


Julia Chui, PT, DPT

Julia graduated from Western University of Health Sciences and is a certified LSVT BIG and Credentialed Clinical Instructor with APTA. She has two years of clinical experience in inpatient/acute rehabilitation unit, OP ortho/neuro and specializes in neuro and vestibular rehabilitation.


Kenneth Hofeldt, PTA
Citrus Valley Health Partners

Kenneth graduated from Mount Saint Mary’s University. He has 18 years of clinical experience as a PTA, primarily in orthopedics, but has spent time in inpatient and acute rehab settings. He has also had courses in NDT, total joint rehab, TPI Level 1 Certification and has recently completed training in Hawk Grips IASTM.


Samuel Garcia, PTA
ARC Physical Therapy – West Covina

Samuel graduated from Loma Linda University and has seven years of experience in an outpatient orthopedic setting. His special interest includes sports biomechanics, performance training and promoting a healthy active lifestyle.


Alison Rae Najera, DPT
Citrus Valley Health Partners

Alison graduated from Chapman University with Academic Honors and has three years of clinical experience in both adult and pediatric physical therapy. Her experience has primarily been in outpatient orthopedics, aquatic therapy, inpatient, acute rehabilitation unit and pediatric school-based physical therapy. She has led treatments with a variety of diagnoses, including orthopedic injury, neurologic disorders, cerebral palsy and children with developmental delays. Alison’s professional interests include strength and conditioning, prehab for competitive athletes and promoting preventive health and wellness awareness.


Angie J. Olea, PTA
ARC Physical Therapy – Glendora

Angie graduated from California State University, San Bernardino with a BA in Kinesiology and participated in the Loma Linda University Physical Therapist Assistant Program. She has four years of experience in orthopedic injuries, pre- and post-op, sports, industrial injuries, functional training and home education. She assists with instructing physical therapist assistant students from various universities and on her personal time she enjoys spending time with friends and family.


Alyssa Peluffo, PT, DPT
ARC Physical Therapy – Chino

Alyssa graduated from California State University, San Marcos with a BA in Kinesiology. She has one year of experience in orthopedic injuries, preand post-op, sports and industrial injuries, manual therapy, spinal, extremities, shoulder, knee dysfunctions, foot and ankle injuries, hand rehabilitation, functional training and home education. Along with caring for her patients, Alyssa enjoys spending time with her family, camping and playing recreational soccer.


Wendy Marrufo, PT, DPT

Wendy holds a Doctorate in Physical Therapy from Samuel Merritt University, Oakland, CA and a BA in Human Biology with a concentration in Exercise Physiology from Stanford University. She has two years of licensed experience in outpatient orthopedics and pediatrics, inpatient acute care and inpatient rehabilitation for adults with orthopedic and/or neurologic dianoses. In her spare time, she continues to give back by participating as a Community Ambassador and in various stroke prevention and total joint replacement community events.


Kumi Kiso, MPT

Kumi graduated from Western University of Health Sciences. She has more than 22 years of experience in outpatient and acute inpatient care. Her specialty includes treating orthopedic and neurologic diagnoses, including joint replacement and various orthopedic and workers compensation injuries.


Ruben Munoz, PTA
ARC Physical Therapy – Chino

Ruben graduated from California State University, Fullerton with a BA in Kinesiology and has completed the Cerritos College Physical Therapy Assistant Program. He has experience in orthopedic injuries, pre- and post-op, sports and industrial injuries, soft tissue mobilization shoulder, knee dysfunctions, foot and ankle injuries and rehabilitation, functional training and home education. He also assists with direct patient care, training new staff, performing isokinetic strength testing and IPCS testing for pre-employment screening for local companies. In his spare time, he enjoys the outdoors and motorcycle riding.
